Membership Application Forms

There are 3 methods to submit your membership application form as follows:
 Option #1 - Online Membership Application Form
 Option #2 - Email Membership Application Form
 Option #3 - Hardcopy Membership Application Form
  • The easiest way to submit your application for membership is to fill out
        Option 1 -  Online Membership Application Form, and submit it electronically. 
  • Option 2 - Email Membership Application Form involves sending the required information via email. 
  • Either Option 1, or Option 2 are by far the quickest way to expedite your application approval process since the information is sent directly to all of the club officers that process your application. 
  • With either of these options you would only be required to mail your check to the treasurer (refer to Club Members Dues Below).
  • If you are not comfortable with computers, or have basic skills you can use Option #3 - Hardcopy Membership Application Form to print out a copy of the form.  Your completed form and annual dues payment will need to be mailed to our club treasurer.
  • Options #2 and #3 are at the bottom of this page.  
  • Club Membership Dues information and mailing instructions are shown below.

Club Membership Dues
  • Our annual membership dues are $45.00
  • Regardless of how you submit your application, dues must be sent to the following address: 
       Woodward Tri Five Cruisers
       3561 Devonshire St., Sterling Heights, MI 48310
  • Checks should be made out to:  Woodward Tri Five Cruisers or WTFC

#1 Online Membership Application Instructions & Form

  • Please read the Club By-Laws before you submit this application form. 
  • Click on the Red Button to open the Club By-Laws:

  • Required fields are marked with a RED asterisk *
  • When you have completed the application, follow the instructions at the bottom of the form.

    Please look at the form you have completed to make sure you have filled in all of the required fields marked with a RED asterisk *.  Also, check your email address to ensure you have entered it in the correct format.  Example:

    If you are satisfied that the form is filled out correctly, press the "Submit Your Membership Application" button.

If you have filled out the form correctly, after you press the "Submit Button", you will receive the following confirmation message:

Thank you.  Your application was submitted successfully!  We will get back to your soon!

Note:
  • This confirmation message will appear under the "START HERE" sign above.  Your application form will also disappear from the screen indicating that the system has accepted your application.
  • If you "did not" receive this message, you will need to return to the form and check the entries again to make sure all of the "mandatory fields" are filled in.  Your information will not be stored in our system, and we will not be able to process your application until you receive the confirmation message.

#3 Hardcopy Membership Application Form
  • A hard copy version of the Membership Application Form is available.  Click on the button below to print or save a copy of the form.
  • Your completed form and annual dues payment will need to be mailed to our club treasurer.  Instruction are on the form.
Note:
  • You also have the option to scan the "completed" form, and attach it to an email.
  • The email address is:  membership@woodwardtrifivecruisers.com
  • In this case, you would only need to mail your check to the treasurer. 
  • This method also expedites the processing of your application:
    • It is automatically stored in our system.
    • All members of the club committee receive a copy when the email is sent.
